Patient Information

The first step towards receiving treatment at my practice is to complete the new patient intake form located on the forms tab. Once you have filled out this screening form, I will review it and get back to you within 1 week. Please note that I am currently seeing patients between the ages of 18-65.

 

Once you have completed the intake form, you will be offered an appointment either in person or via Telehealth. Once an appointment is made, I will send you a link to download MYIO which is a portal that we will use to communicate with each other.

Currently, this practice is accepting cash/credit card payment only. I apologize for the inconvenience, and hope to set up insurance collaboration in the near future. The fees for service are as follow:

Initial Psychiatric Diagnostic Assessment Fee: $200.00

Follow up visit Moderate Complexity Fee: $150.00

Follow up visit Low Complexity Fee: $100.00
